Побочные явления карбамазепина, так же как и побочные явления многих противоэпилептических средств, включают в себя кардиотоксические эффекты, такие как атриовентрикулярная блокада, брадикардия и нарушения ритма сердца. Тем не менее, у карбамазепина также описано наличие антиаритмического, нормотимического и мембраностабилизирующего действия, что обусловливает применение препарата с целью лечения нарушений сердечного ритма у детей. По данным литературы, известно применение карбамазепина в качестве антиаритмического препарата в тех случаях, когда базисная терапия оказалась неэффективной. По этому показанию препарат не зарегистрирован нигде в мире, и, таким образом, он применяется вне официальной инструкции по применению. Целью данного обзора литературы является анализ и обобщение существующих данных о действии карбамазепина на сердечно-сосудистую систему, определение степени безопасности применения карбамазепина в качестве антиаритмического препарата, а также описание различных факторов, способствующих развитию его побочных явлений.</p></abstract><trans-abstract xml:lang="en"><p>Carbamazepine is an anticonvulsant that is commonly used in neurological and psychiatric patients to treat epileptic seizures, neuropathic pain, or bipolar disorder. Carbamazepine side effects, as well as side effects of many antiepileptic drugs, include cardiotoxic effects such as atrioventricular block, bradycardia, and cardiac rhythm disorders. However, carbamazepine has also been reported to have antiarrhythmic, normotimic, and membrane-stabilizing effects. This results in its administration to treat arrhythmias in children. Based on literature, carbamazepine administration as anti-arrhythmic drug is known in cases where the basic therapy was ineffective. The medication is not registered anywhere in the world for this purpose. Thus, it can be administered only off-label.
