Самой эффективной профилактикой,  существенно снижающей заболеваемость, является вакцинация против ротавирусной инфекции (РВИ).</p><p>Цель исследования — оценка эффективности затрат на массовую вакцинацию детей 5-валентной вакциной против РВИ в Российской Федерации.</p><sec><title>Методы</title><p>Методы. Оценка осуществлялась с помощью моделирования на основе опубликованных  данных по эффективности вакцины и эпидемиологических показателей по РФ. В базовом  варианте анализ проводили с позиции общества в целом. При проведении анализа  чувствительности оценивали также эффективность затрат с позиции системы здравоохранения. Оценку осуществляли на период дожития вакцинированных детей.  Затраты на терапию РВИ соответствовали тарифам системы обязательного медицинского страхования по Санкт-Петербургу на 2017 г. Цена 1 дозы вакцины при расчете составляла  1450 руб. Затраты и продолжительность жизни с учетом качества дисконтировали на 3,5% в год.</p></sec><sec><title>Результаты</title><p>Результаты. С учетом принятых допущений массовая вакцинация позволит предупредить в вакцинированной популяции в среднем 4675 случаев амбулаторной РВИ и 1732 случая РВИ,  требующей госпитализации, в расчете на 100 тыс. детей первого года жизни при охвате  90%, в невакцинированной популяции — 4128 и 1212 случаев соответственно.  Прогнозируемый объем предотвращенных затрат в целом — 2,94 тыс. руб. в расчете на 1  ребенка (54% — в вакцинированной когорте, 46% — в невакцинированной). Эффективность затрат при оценке с позиции общества составит в целом 260,1 тыс. руб. в расчете на  дополнительный год жизни с учетом ее качества (QALY), а при оценке с позиции системы  здравоохранения — 653,0 тыс. руб./QALY. Таким образом, в обоих случаях коэффициент  эффективности дополнительных затрат на вакцинацию против РВИ не превысит  общепринятого порога готовности платить, равного утроенной величине валового  внутреннего продукта в РФ (по данным за 2016 г. — ~ 1,76 млн. руб.). Прогнозируемая экономическая эффективность выборочной вакцинации в 4,94 раза ниже, чем вакцинации массовой.</p></sec><sec><title>Выводы</title><p>Выводы. Массовая вакцинация детей 5-валентной вакциной против РВИ позволит не только снизить заболеваемость в РФ, но, с учетом принятых допущений, может также  рассматриваться как экономически эффективное вмешательство.</p></sec><sec><title>Источник финансирования</title><p>Источник финансирования.Опубликовано при поддержке компании МСД</p></sec></abstract><trans-abstract xml:lang="en"><p>One of the main causes of the incidence of intestinal infections in children under 5 years infection is rotavirus. Vaccines against rotavirus infection substantially reduce morbidity.</p><p>The objective of this study is to analyze cost-effectiveness of vaccination of children with 5-valent vaccine against rotavirus infection in the Russian Federation.</p><sec><title>Methods</title><p>Methods. The evaluation was carried out using modelling on the  basis of published data on the effectiveness of vaccines and  epidemiological data for the Russian Federation. In the base case analysis was performed from the position of the society as a  whole. During the sensitivity analysis cost-effectiveness was also  evaluated from the position of the health system. The evaluation was performed on the period of survival of vaccinated children. The costof therapy corresponded to the rates of OMS in St. Petersburg in 2017, the price of 1 dose of vaccine accounted for in the calculationof 1450 rubles. Costs and life expectancy with regard to quality were discounted at 3.5% per year.</p></sec><sec><title>Results</title><p>Results. Taking into account the accepted assumptions, the mass  vaccination will prevent the vaccinated population in the average  4675 RVI outpatient cases and 1732 RVI cases requiring  hospitalization per 100 thousand children the first year of life, with  90% coverage. In unvaccinated populations will be prevented 4128  outpatient cases and 1212 RVI cases requiring hospitalization. The  projected amount of avoided costs in general — 2.94 thousand RUB  per 1 child (54% in the vaccinated cohort, 46% in unvaccinated  population). Cost-effectiveness will be in the evaluation from the  perspective of society as a whole of 260.1 thousand RUB counting on an quality adjusted life year (QALY), and the evaluation from the perspective of the health system — 653.0 thousand roubles/QALY. Thus, in both cases, costeffectiveness of rotavirus vaccination per 1  QALY will not exceed the generally accepted threshold willingness-to- pay equal to three times the gross domestic product in Russia (2016  — 1.76 million RUB). The projected economic efficiency of selective  vaccination 4.94 times lower than one of mass vaccination.</p></sec><sec><title>Conclusions</title><p>Conclusions.
