При этом стартовая схема терапии должна оставаться эффективной и безопасной при длительном применении.</p><sec><title>Цель исследования</title><p>Цель исследования: изучить эффективность и безопасность различных схем стартовой ВААРТ у детей с ВИЧ-инфекцией.</p></sec><sec><title>Методы</title><p>Методы: в рандомизированное контролируемое исследование включали детей в возрасте от 1 до 3 лет с ВИЧ-инфекцией. Клиническое, иммунологическое и вирусологическое обследование проводили до начала и через 12 мес ВААРТ. Стартовая схема терапии включала 2 нуклеозидных ингибитора обратной транскриптазы ВИЧ — зидовудин и ламивудин. Детей рандомизировали на прием третьего компонента терапии — лопинавир/ритонавир (n = 25) и невирапин (n = 23).</p></sec><sec><title>Результаты</title><p>Результаты: в группу лопинавир/ритонавир рандомизировано 25 пациентов, в группу невирапина — 23 ребенка. Через 12 мес лечения подавление репликации ВИЧ (вирусная нагрузка крови &lt; 50 копий/мл) было достигнуто у 25 (100%) больных в группе лопинавира/ритонавира и у 16 (70%) — в группе невирапина (р = 0,003). При использовании обеих схем ВИЧ-инфекция клинически не прогрессировала. При применении лопинавира/ритонавира медиана числа CD4+СD3+ лимфоцитов увеличилась с 20,5 (12; 23) до 30% (27; 34) (р &lt; 0,001), при лечении невирапином — с 21,5 (17; 23) до 29% (27; 38) (р &lt; 0,001). Нежелательные явления развились у 13 (27%) из 48 пациентов обеих групп, участвовавших в исследовании. При приеме лопинавира/ритонавира у 3 детей возникла тошнота, у 2 — рвота. При приеме невирапина у 1 больного отмечено появление аллергической сыпи, еще у 1 развился лекарственный гепатит.</p></sec><sec><title>Заключение</title><p>Заключение: высокая эффективность и безопасность лопринавира/ритонавира позволяют рекомендовать препарат в качестве третьего компонента для стартовой схемы ВААРТ ВИЧ-инфицированных детей.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: Adverse course of HIV infection, rapid development of immunosuppression, severe opportunistic infections and malignant tumors and high mortality substantiate the importance of timely prescription of highly active antiretroviral therapy (HAART). The initial therapy pattern must remain efficient and safe in the long term.</p></sec><sec><title>Objective</title><p>Objective: Our aim was to study effectiveness and safety of different patterns of initial HAART in HIV-infected children.</p></sec><sec><title>Methods</title><p>Methods: The randomized controlled trial involved HIV-infected children aged 1–3 years. Clinical, immunological and virological examinations were conducted before and 12 months after HAART. The initial therapy pattern included 2 nucleoside HIV reverse transcriptase inhibitors — zidovudine and lamivudine. Children were randomized to the groups differing in the third therapy component: lopinavir/ritonavir or nevirapine.</p></sec><sec><title>Results</title><p>Results: 25 patients were randomized to the lopinavir/ritonavir group, 23 children — to the nevirapine group. After 12 months of treatment, HIV replication suppression (blood viral load &lt; 50 copies/ml) was achieved in 25 (100%) lopinavir/ritonavir group patients and in 16 (70%) nevirapine group patients (p = 0.003). HIV infection did not progress clinically regardless of the pattern. The median number of CD4+CD3+ lymphocytes in the lopinavir/ritonavir group increased from 20.5% (12; 23) to 30% (27; 34) (p &lt; 0,001), in the nevirapine group — from 21.5% (17; 23) to 29% (27; 38) (p &lt; 0.001).
